This booklet provides information on salary packaging a novated lease. It has been written specifically for you as an employee of the Queensland Government.

Statewide Enquiry Form

This form is used by existing and potential clients to make enquires via the Statewide website.

Statewide Finance Application Form
When a potential client wishes to proceed after receiving a novated lease quote, this form is sent to the potential client to complete, sign and return. It is used to obtain vehicle finance pre-approval.

Financier Privacy Act Form
This form is sent to the potential client along with the Statewide Finance Application Form. The finance application cannot be processed without receipt of this signed form.

Statewide Novated Lease Application Form
This form is used to confirm a new client's personal details, vehicle details, the capital value of the car, budgeted running expenses and pre/post-tax payroll deductions.

Statewide Amendment Form
This form is used to vary an existing arrangement and increase or decrease payroll deductions. The form is also used when replacing one novated lease with another to amend payroll deductions. When Statewide completes this form with any change in payroll deductions, the effect on take home pay will be disclosed to the client prior to implementing any change.

Third Party Authority Form
This form is required if a client wishes to authorise a partner/financial adviser/etc to obtain information from Statewide regarding the client's novated lease account.

Statewide Authority Form
This form authorises Statewide to contact the client's salary packaging administrator as required. It is sent to the client with an explanatory statement and a request to return it completed and signed.

Statewide Update of Contact Details Form
This can be completed in a paper format or online within the client's online Statewide account. By simply logging in, using supplied User ID and user-set password, client's can login to their online account and update their address, phone numbers or preferred email address. This is a quick and simple process.

Statewide Cessation Form
This form is used when a client's lease is maturing or has been paid out. It provides Statewide with the authority and necessary information to finalise the lease and close the client’s lease account.

Statewide Reimbursement Claim Form
This form is submitted by a client to claim a reimbursement for an eligible novated lease expense. The client provides relevant details, proof of payment and a signed declaration (the Statewide Reimbursement Claim Form).

Change of name form
Change a client's name must only be done by completing the Change of Name form and submitted to Statewide along with required supporting documentation.
